Not recommended the combination of sildenafil with other drugs for the treatment of sexual dysfunction in patients with anatomical penile deformity (cavernosal fibrosis, calcification, angulation) or conditions that predispose to priapism (leukemia, myeloma, sickle cell anemia). Do not know the effects that the drug might have on patients with retinitis pigmentosa inherited bleeding disorders, gastrointestinal ulcerative disease, and should therefore be administered with caution in these cases. The drug does not protect against AIDS and sexually transmitted diseases, and therefore the care recommended in this situation must be strictly followed.

Interactions.

The therapeutic agents that are potent inhibitors of cytochrome P-450 3A4 as erythromycin, ketoconazole and itraconazole may increase plasma levels of sildenafil. Thus, in patients taking these drugs is recommended starting dose of 25 mg. Similar situations occur in advanced age (> 65 years), cirrhosis and severe renal impairment. In vitro, sildenafil potentiates the antiplatelet effect of sodium nitroprusside (NO donor agent). When administered together, cimetidine can increase plasma concentrations of sildenafil. There is no record of pharmacological potentiation with alcohol, aspirin, tolbutamide, warfarin, thiazide diuretics, calcium antagonists (amlodipine) or antidepressants (tricyclics or IRS). The area under the curve (on a graph of concentration vs. time) showed an increase of the active metabolite according to the association with non-selective blockers, potassium-sparing diuretics and loop diuretics, but without clinical significance.

Contraindications.

Known hypersensitivity to the drug. Children and women (pregnant or not). Agents nitrite or NO donors (nitroprusside).
